first, I always have a checkbox with 'alert on reply' under comments, and I don't want this, how to remove

secondly, in custom template, first line correspond to type->comment->comment title , but the problem is that I have this title 'comments'  on top of article and the comment is on bottom ! what to modifie for disable or remove comment title in type ?
